MEDICAL FORMS
To fully complete registration in PowerSchool, you will need to provide some medical forms in collaboration with your doctor. Here are the forms you will need:
Non-Prescription Authorization
Preparticipation Physical Exam
PPD Documentation (Required for juniors/only required for seniors who travel outside of the country between May-August)
Prescription Authorization Form (One form must be submitted if you take any prescription medication)

ATHLETICS
To participate in any sports at GSSM, students and parents or guardians will need to create a BigTeams account. Information about setting that up can be found here.
