MEDICAL FORMS
To fully complete registration in PowerSchool, you will need to provide some medical forms in collaboration with your doctor. Here are the forms you will need:
GSSM Medical Exam*Required
Non-Prescription Authorization*Required
Preparticipation Physical Exam*Required
PPD Documentation (Required for juniors/only required for seniors who travel outside of the country between May-August)
Prescription Authorization Form (One form must be submitted for EACH prescription medication)
